Advanced Directives - Health Info Authorization
In order to prepare to acquire medical information in an emergency to help your spouse/family members, you will be required to
have an "Authorization to Share Protected Health Information" document on file with your health
insurance carrier.
Getting this done ahead of time can avoid all sorts of inconvenience and frustration
when you least need it. You can allow more than one person to gain access to the information and for certain periods
of time that you determine and note on the forms. You can also designate different people to have access to different types
of information and for different time periods as well.
Of course this would require multiple forms. you
can make this as simple or complex as you want it appears....if you don't have complicated issues you are in luck and
can make a simple designation that would allow someone to get the information you might need to provide other medical providers
in an emergency. Having you own medical file backup copied or digitized can be very helpful....if not time consuming.
Better now than later..or after the records have been dispersed by the wind across several counties.
CHILDREN For the most part, until a child reaches 18, parents can access most of their
child's health information without permission - but cannot have access to diagnoses, treatment or payment info regarding
venereal disease, abortion, drug and alcohol abuse without the child's release. You can decide how much of an impact
these conditions have or might have on your ability to protect your children in an emergency and act accordingly. Better
to be aware and not surprised later.
HIV/AIDS Different
authorization forms exist for HIV/AIDS information based on your state laws and should be available through your state's
health department online.
Contact your health insurance carrier for the required forms.

New ICE Cards
ICE4SAFETY has decided that handling the sale of cards/stickers and such should be done here through the website
to improve communications with end users and generate some money - however small to help defray the cost of this public service.
The pictured UV coated ICE Card is sleeved in an adhesive backed plastic pouch that can be used to secure
it to helmets, refrigerators, automobile glove boxes and other flat or even slightly curved surfaces.
We will be offering these ICE Cards along with our ICE Stickers in varying quantities to those interested in purchasing
for small groups or families. We have kept the prices low and such that they still allow us to process and ship
them to you in these lower quantities. Thank you for your continued patience and support.

ICE Sticker - Outdoor Testing
Tests on the ICE Stickers in the outdoor environment have
been quite favorable. We have been monitoring the effects of the sun and weather over the past 6 months on stickers
placed randomly on the reverse of street signs that predominately face the sun. We have also placed some on vehicles
for several months and can report that they do not appear to have been affected by UV or the weather.....we will leave them
on during the winter months to see how well they fare....chances are they will do just fine.
